Support Services Planning

Each of our service user is an individual with very different life experiences.
This means the types of support required will vary from one young person to the next.

Regardless of the challenge, our aim is to ensure that each one of our service users is supported in a way that their full potential is maximised and achieved.

In order to do this, we work very closely with the referring borough..On accepting responsibility to support a young person, we request a copy of the young person’s pathway plan. In conjunction with the allocated social worker and the young person, a support plan is agreed and expectations are discussed.

Monthly Progress reports are produced in accordance with the Quality Protects guidelines. The report details work done with the young person, tasks set and levels of achievement as well as identifying areas requiring further. Each service user is supported in registering with the necessary health professionals such as GP, dentist and optician. They are encouraged to keep and electronic/manual diary for ensuring appointments are not missed.

Where necessary, we are able to advise on benefits that may be available to the young person.
We discuss individual aspirations and support the young people in getting access to education or employment. We encourage pursuit of education as a stepping stone to employment rather than a dependence on state benefits.
